Provider Portal

Welcome to CBHNP’s Provider Portal!

You may register as many staff as you wish for access to this portal. Because this portal is based on individual email addresses, each person needs to register seperately using their email address in order to receive updates via email.

To keep your account current, use the My Account tab located at the top of each page once you login into the Provider Portal. In this section you can make changes to existing information such as your address and phone number, along with selecting different Levels of Care that you would like to view and receive information for.

If you forgot your password, use the Forgot Password feature at the bottom of this page. A new password will be sent to your email address in a couple of minutes after completion.
